Westpoint requires all agents and agencies receiving commissions from us be licensed in the state they are doing business. You will also be required to submit a W-9 for commissions payable from us. You can download a W-9 form by clicking this link:

Additional documents required to write Accident Medical Health business are:
- Copy of agent’s Accident & Health License for the state in which the insured is located.
- Copy of the agency’s Accident & Health License for the state in which the insured is located
- Copy of a current E&O Declarations page
- Producer Questionnaire

- Copy of agent's Accident & Health License for the state in which the insured is located
